Forms

There are a variety of forms to assist with planning and completing your program.  In this section we present links to commonly-used forms.

Graduate Forms

    Master's Degree and PhD students will need to fill out at least these forms for the Graduate School (be sure to use the current forms):

    • Program of Study Form (after 12 hours of class work)
    • Examination
    • Application for Degree

    You may also need

    • Change of Advisor
    • Change of Program
    • Transfer Credit

    All Graduate School forms can be found on the Graduate School Forms Web page.

    The Klipsch School also has graduate record check forms to ensure that the program follows program guidelines.

Undergraduate Calculator Policy

    Faculty members in the Klipsch School may choose one of the following two calculator-use policies for in-class exams and quizzes administered in their core ECE classes:

    • All electronic computing devices are banned
            or
    • Students may use one of the approved calculators on the list provided by the National Council of Examiners for Engineering and Surveying available here: NCEES 2010 Calculator Policy

    The faculty member’s choice shall be communicated as part of the syllabus and in other appropriate ways for the class.
